Is Dried Blood Spot Testing Widely Used for Anti-Doping?

Dried blood spot (DBS) testing is a recent addition to the anti-doping toolkit, offering a less invasive and more efficient way to collect samples from athletes. While it is not yet as widely used as traditional urine or venous blood testing, DBS has gained significant traction in recent years. Endorsed by the World Anti-Doping Agency (WADA) and implemented at major international competitions, DBS represents a shift toward more accessible and flexible approaches to doping control.

WADA Approval and Initial Adoption

In 2021, WADA officially approved DBS as a valid sample type for doping control. This decision followed years of validation studies confirming its reliability for detecting prohibited substances. Advantages Driving DBS Adoption

The appeal of DBS lies in its practical advantages. For athletes, it is far less invasive, requiring only a small finger-prick sample instead of a full venous blood draw. This reduces discomfort and makes it possible to collect samples immediately after competition without waiting for recovery. For doping control officers, DBS simplifies logistics: dried samples are stable at room temperature, easier to ship than liquid blood, and can even be mailed without refrigeration. Furthermore, DBS collection does not require a phlebotomist, meaning the same officers who collect urine samples can also handle DBS collection. Finally, each DBS requires only a few drops of blood, a fraction of the volume needed for conventional blood testing.

Current Limitations

Despite these strengths, DBS has not replaced traditional methods. Certain substances are still better detected in urine or require larger plasma volumes than DBS can provide. At present, athletes cannot opt for DBS in place of a mandated urine or venous blood test; the decision rests with testing authorities. Additionally, while WADA-accredited laboratories are rapidly expanding their analytical capabilities, not all substances can yet be fully validated for DBS analysis. These limitations mean DBS functions as a complementary, rather than standalone, testing method.

Outlook for Wider Adoption

The trajectory of DBS in anti-doping suggests continued expansion. WADA has published technical documents to standardize collection and analysis procedures, and national agencies are beginning to integrate DBS into regular testing regimes. As laboratory methods mature and success stories from major competitions accumulate, DBS is expected to become a fixture of anti-doping programs worldwide. Future possibilities include more frequent monitoring, enabled by the convenience of DBS, and new applications such as detecting emerging doping strategies, including genetic manipulation.
